Connecting Futures has strong working relationships across local NDIS service providers, employers, mainstream and community services, though our diverse networks we can provide Support coordination to:

Agency managed NDIS participants with Support Coordination - Coordination Of Supports (COS) or Specialist Support Coordination (level 3) in their plan

Plan Managed NDIS participants with  Support Coordination - Coordination Of Supports (COS) in their plan

Self-Managed clients who have goals around capacity building to increase independence in their plan

When selecting a support Coordinator it's important both parties are clear on expectations and a strong working relationship is built.

Connecting Futures approachable Support Coordinators are highly skilled, having experience in:

Working with participants on the Autism spectrum Disorder (ASD)

Equipment & Home Modifications  (consumables, Assistive Technology &   Home Modification)

Complex support coordination across funded and mainstream services

Employment (Supported and mainstream)

Independent living goals

Capacity building (working to help build independence by linking with funded and mainstream services)

Working with participants living in the Loddon Murray region
